Care Coordinator
FULL TIME | PART TIME
Pinnacle care providers is always accepting resumes for Registered Nurses and Licensed Practical Nurses. You will be part of a team that provides the highest quality care to patients and residents. The Care Coordinator is responsible for coordinating care between our physicians and advanced practice nurses.
Responsibilities
-
Facilitate communication between physicians, advanced practice nurses, nursing facility, patient, and patient’s family
-
Collect nursing assessment data and communicate effectively
-
Gather and document data from nursing staff and patient’s medical record to complete H7P, progress notes, and/or discharge summary

Transcribe verbal orders given by the physician or advanced practice nurse

Accurately bill patient visits in a timely manner
